AHA has incorporated Google Forms to perform the registration process online.

If you're interested in having your child be an Honorary Captain at one of our participating schools' Varsity High School games or one of our many sponsored AHA Fun events, please fill out our Athlete Waiver Form - be sure to click submit at the bottom of the page!

Submitting a signed and dated Waiver Form is a requirement prior to taking part in these events. This waiver is good for the entire scholastic year. You only have to fill this out once per calendar school year!!
